CenClear maintains certain equipment to serve the mission of CenClear and for use by employees to perform their job functions.
This form allows the use of CenClear equipment, by the name above. The name above will be responsible for taking the necessary precautions to protect the equipment and to store it in a manner that provides adequate protection when it is not in use, thus not subjecting the equipment to possible theft or damage. If it is determined that loss or damage is a result of negligence, the borrower may be held financially responsible for the repair or replacement of the equipment.

Terms and Conditions:
- CenClear equipment are not permitted for personal use or gain.
- The CenClear employee assumes liability for the equipment and accessories related to the equipment.
- Staff must sign this form before any equipment can be given to the employee for work use.
- Employees may request a copy of this form.
- If the equipment is not working properly or broken, report to the IT department immediately. (Extension2345 or [email protected])
- Users may be required to pay equal to the replacement cost if the equipment is lost, stolen, or not returned.
- Staff will be assessed the following approximate amounts for lost, broken or stolen items:
- Laptops, Tablet Computers, iPads, Cell Phones (est. $1600, charges based on actual repair)
- Power Adaptors (est. $60, charges based on actual replacement cost)

I acknowledge that while I am working for CenClear, I will take proper care of all company equipment that I am entrusted with. I further understand that upon termination, I will return all property of CenClear in proper working order. I understand I may be held financially responsible for damaged or unreturned property. This agreement includes, but is not limited to, the following: (laptops, cell phones, tablet computers, other equipment). I understand that failure to return equipment will be considered theft and may lead to criminal prosecution by CenClear.
